In order to help you prepare your proposal submission, below are the fields that will be required when completing the form.

  1. Project lead name and department
  2. Project lead contact information
  3. Project title
  4. List the name and department of project collaborators
  5. Describe your proposed project, including specific goals and outcomes
  6. The project has an impact at which of the following levels? (select all that apply)
    • One course
    • Two or more sections of the same course
    • Two or more different courses in the same department -
    • Program or department level
    • Multiple departments
  7. Describe the impact this project would have on teaching and learning.
  8. Describe any background work done related to this project.
  9. Describe the specific activities that would be covered by this grant.
  10. Briefly describe how the project will be assessed.
  11. Attach a detailed budget and timeline for your project.
  12. Attach a letter of support from your Department Chair or Program Director
